Management of Caries, Erosion and Hypersensitivity Risk with a Saliva Biomimetic and Stannous Fluoride

Casein phosphopeptide amorphous calcium phosphate (CPP-ACP), a saliva biomimetic derived from bovine milk, is in a variety of oral care products (eg tooth crème, dental varnish and GIC) and has been shown to remineralise and regress early stages of enamel and dentine demineralisation. Recently CPP-ACP has been shown to interact synergistically with stannous fluoride (SnF2) to stabilise and enhance delivery of stannous ions (Sn2+) and fluoride ions to produce a superior remineralisation system for both enamel and dentine. The scientific evidence and clinical application protocols for the different CPP-ACP and F combinations and products will be presented.

Gingivitis is associated with plaque accumulation, resulting in inflammation, an increase in microbial diversity and soft tissue swelling to form a pocket. A saliva biomimetic CPP-ACP has been shown to increase plaque commensal/beneficial bacteria and significantly reduce periodontal disease. This was closely associated with an increase in commensal species, (particularly periodontal pathogens antagonists) and to suppress inflammation and induce mucosal tolerance. In conclusion, modulating plaque microbial composition by increasing the fitness of commensal/beneficial bacteria can improve mucosal homeostasis and periodontal health.
